Nurse > Nurse > OH&S
This screen records when an OH & S incident has occurred at a where the Nurse had some involvement, even if only recording the incident.
Adding an Incident
To add an Incident, Select "Insert" on the OH&S tab and complete the following detail
OH&S Incident tab: -
- Incident ID – System generated number which cannot be changed
- Incident Date and Time – The date and time the incident occurred
- Report Date and Time – The date and time the incident was reported
- Last Updated, Created, Last Updated User and Created User are system generated and cannot be changed
- Incident Summary – Drop-down list of OH&S incident types which can be added and edited (you cannot delete an OH&S Summary Type) from Data Entry > Types> OH&S Summary Types
- Hospital Details – Drop-down menu lets you select the hospital where the incident occurred. Once selected, the rest of the hospital details self-populate.
Prior to Incident Tab
- Free text field where you can describe the situation prior to the incident occurring.
During Incident Tab
- Free text field where you can describe the situation during the incident.
Post Incident Tab
- Free text field where you can describe the situation post the incident.
First Aid Tab
- Free text field where you can describe the first aid administered.
Item/Action Tab (use the tab button to navigate through this screen)
- Assigned User – Person responsible for resolving the issue
- Status – Status of the incident. These can be added to or edited under Data Entry > Types > OH&S Status Types (you cannot delete an OH&S Status Type)
- Created User, Created Date and Time, Last Updated User and Last Updated Date and Time are system generated fields and cannot be changed.
- Action Item Description – Free text field where you describe the action required
Notes Tab
- Note Timestamp and User ID are system generated and cannot be changed
- Note Type – Drop-down menu from selection defined under Data Types > Types > Note Types
Linked Documents Tab
- Allows you to add document attachments relating to the incident
Nurses Tab
- Allows to you add Nurses who were involved in the incident either directly or indirectly
